Job description

Your PhD project will focus on in and ex-vivo modelling and testing to enable optimized SIRT. It is our goal to translate the outcomes of this research project into a new calculated and better predicted treatment option for patients. Will you join our ambitious team? 

The main goal in this project is to develop a method to optimize SIRT by investigating and ultimately altering the parameters that affect the distribution of the microspheres. The exact distribution of microspheres will be investigated in silico, ex vivo and in vivo, painting a complete picture of the mechanisms underlying SIRT. During the research we will comprise a survey and a brainstorm session with key opinion leaders in SIRT to collect possible parameters that can influence hepatic blood flow and/or microsphere distribution. At the start of the project analyses of the hepatic blood flow of healthy volunteers and patients will be performed. You will identify and segment the liver and its blood vessels (both the hepatic arteries and portal veins) based on Computer Tomography (CT) and Magnetic Resonance images (MRI)

Based on the survey and analysis you will collaborate with the other PhD candidate on the planning of 3d-printed hepatic vessel models. Your main tasks will be the set-up of an ex-vivo liver model. The main goal of the research is the testing of different microsphere sizes, numbers and compositions under imaging guidance to gain detailed information around the microsphere flow and distribution. Injection speed and catheter tip position (of different catheters) will be investigated in relation to the microspheres distribution. In the next steps ex-vivo perfused pig livers will be investigated with a selected amount of parameters. However, the similarity to the actual situation in the patient is limited.

Therefore, based on the results within the pig livers the imaging schedule and set-up of testing the parameters in ex-vivo human livers will be planned. The flow dynamics will be quantified with MRI or US. This setup and performance of the preclinical and clinical research will be part of your work. Analysis of human perfused ex-vivo livers will be performed in close cooperation with the department of surgery in the University Medical Center Groningen. Donor livers that were selected for liver transplantation but did not met the criteria for transplantation will be used. Liver perfusion of these livers is applied to simulate the real physiological situation in humans. Also, explanted livers with hepatocellular carcinoma, obtained during liver transplantation, will be used.

Finally, patients with liver tumors will be treated and the specific parameters will be investigated. Administration of the microspheres will be performed in fractions. All outcomes will be used to validate the computational planning system, the validated computational planning system is the final goal of this project.

Tasks and responsibilities

  • Analysis of scientific data and the set-up of list of parameters that will be investigated.
  • The research task will mainly focus on the development of an image guided ex-vivo and in-vivo animal and human model for testing the influence of blood flow and microsphere characteristics on the distribution of microspheres.
  • Validating the mathematical model on Ex-vivo human liver and patients.
  • Valorisation to the end users in collaboration with two other PhD candidates on this project
  • Write scientific publications and present findings at (inter)national scientific meetings.
  • Work in a team with other scientists and clinicians to plan, discuss, and perform research in a stimulating environment.

The department has extensive expertise in imaging and uses state-of the art CT, ultrasound, MRI and nuclear imaging. Advanced MRI facilities for both patient care and (animal experiments) research are available and accessible. Within the Department of Radiology oncology is an important focus of research and patient care. Both pre-clinical and clinical research can be performed in the animal lab (PRIME) and in the Medical Innovation & Technology expert Centre (MITeC), which is a conglomerate of three Operating Rooms (OR’s) in the newly built OR-complex in the Radboudumc.

The project Understandig Liver treatment to improve distribution (Ultimo)
Each year, over 1.5 million people worldwide develop liver tumors. Selective Internal Radiation Therapy (SIRT), also called radioembolization, is a treatment option for patients suffering from liver tumors that do not benefit from conventional therapy. During the treatment, radioactive microspheres are injected into the liver artery that supplies the tumor, where they lodge in and around the tumor and deliver a high local radiation dose. However, the outcomes vary greatly between individuals. It is not well known what factors give rise to these mixed outcomes.

In this project, we will unravel the fundamentals of SIRT, allowing for the development of a validated computational planning system to hand the clinician a tailor-made treatment plan per patient, resulting in optimal patient care and ultimately reaching an effective transfer of knowledge on SIRT from the technical starting point to the end users for whom the technique is applied: the affected patients. The project is funded by The Netherlands Organisation for Scientific Research (division Applied and Engineering Sciences; application number OTP 18427).

You will be positioned in the research group of Dr Frank Nijsen at the department of Medical Imaging, Radboudumc in Nijmegen and you will be part of a dynamic, multidisciplinary team. Within this project you will work together with two colleague PhD candidates located at the University of Twente (Dr. Erik Groot-Jebbink) and Radboudumc. The (pre)clinical studies on human ex-vivo livers will be performed mainly at the department of surgery in the UMCG (supervisor Dr Koert P. de Jong, HPB Surgeon). Preclinical work on pig livers and the validation on patients treated with radioembolization will be performed in the Radboudumc.
