When initiating a sub-biobank, clear information must be available for potential participants, with several essential components. The Radboud Biobank has drafted a template for the Information letter and consent form. This template is used as a standard for review by the CMO Radboudumc/CMO-light. This template is to be used as a guide for both the structure and the wording in the information letter. The template has been created under the advisement of the Patient Advisory Board (PAR), the communication department and the CMO-light.
